Investigating What Went Wrong

It is entirely possible to examine what went wrong while not pointing fingers. You will have to focus more on the how (process) than the why (or the who), and keep your employees focused on this as well. This may require some redirecting now and then. Again, this kind of conversation should be held once any strong negative emotions from the crisis have cooled off.

How did this happen?

Where were the breakdowns?

Remember, placing blame isn’t our purpose right now. I’m less worried about who did what, and more concerned about how things didn’t go as planned.

Was this crisis something that should have been anticipated in any way? What signs did we miss?

Is this something that could happen again? How do we prepare for it?

Who are the key stakeholders in this process or situation?
